If you already deal with TMJ issues, jaw pain, jaw tension, clicking, headaches, or neck and shoulder tension, certain oral sex techniques may become uncomfortable pretty quickly. But that doesn't mean blowjobs are off the table.
It may just mean changing how you give them. And here's the most important thing I want you to remember: A great blowjob does not require taking an entire penis into your mouth. You have lips. You have a tongue. You have hands. You have toys. You have lubricant.
So let’s take the job out of blowjob and find ways to make it feel amazing for them without making your jaw do all the work.
First: TMJ vs. TMD - What's the Difference?
A tiny anatomy lesson before we get into the fun stuff.
Your TMJ is your temporomandibular joint, the joint located on each side of your jaw that allows it to open, close, and move. TMD, or temporomandibular disorder, is the broader term for disorders affecting the jaw joint and surrounding muscles.
People commonly say, "I have TMJ," though technically everyone has TMJs...we just don't all have a disorder involving them.
Symptoms of TMD can include:
- Jaw pain or tenderness
- Pain in the muscles used for chewing
- Jaw stiffness
- Limited movement
- Jaw locking
- Painful clicking or popping
- Headaches
- Pain that spreads into the face or neck
Neck and shoulder discomfort may also occur alongside TMD. So if holding your mouth open for oral sex makes your jaw angry, there's a pretty understandable mechanical reason. The solution isn't necessarily learning how to tolerate more discomfort. The solution is often finding ways to reduce the workload.
1. Don't Start With Your Mouth Open as Wide as Possible
If you have jaw issues, going from zero to maximum mouth opening probably isn't the greatest warm-up. In fact, guidance for TMD generally recommends avoiding overuse of the jaw muscles and avoiding unnecessarily wide opening when it aggravates symptoms.
So start smaller.
Instead of immediately trying to take most of the shaft into your mouth, spend some time focusing on the head.
Try:
- Gentle kisses
- Letting your lips linger
- Slowly licking around the head
- Using the flat part of your tongue
- Gentle suction with your lips
- Alternating between licking, kissing, and sucking
You're creating sensation without keeping your jaw at its widest position.
There's another bonus: You're teasing. And teasing isn't exactly a downgrade.
2. Follow the Golden TMJ Rule: Mouth on the Tip, Hand on the Shaft
If there is one technique I want people with jaw pain to remember, it's this:
Your mouth does not have to cover everything.
Use your hand around the lower portion of the shaft while your mouth concentrates on the head and upper portion. Move your hand and mouth together so the sensations blend into each other. From your partner's perspective, they're still getting stimulation along more of the penis.
From your jaw's perspective? You're not trying to win an Olympic medal in maximum mouth opening.
You can change the ratio depending on what's comfortable:
More jaw-sensitive day?
More hand, less mouth.
Jaw feeling good today?
Maybe you incorporate a little more oral.
There doesn't need to be one technique you perform exactly the same way every time.
3. Try the Melting Popsicle Technique
This is one of my favorite oral sex tricks because it relies heavily on your tongue instead of your jaw.
Pretend the penis is a melting popsicle or ice cream cone.
Your job is to catch all the imaginary drips before they reach your hand.
Use slow licks:
Up the shaft.
Down the shaft.
Around the head.
Along the underside.
Then alternate those movements with kisses or gentle suction up and down the shaft.
You can also wrap your lips around the penis briefly, release, and return to licking.
Think: Lick → kiss → suck → hand → repeat.
Your partner gets tons of sensory variation without your jaw having to repeatedly open and close through a large range of motion.
4. Let Your Tongue Do More of the Work
Your tongue is one of your best tools if your jaw fatigues easily.

Try:

5. Stop Thinking Deeper = Better
This deserves its own section because there is so much pressure around deep throating. You do not have to deep throat someone to give fantastic oral sex.
Period.
If taking more of the shaft into your mouth requires you to open your jaw to the point of pain, there are plenty of other ways to create sensation.
And if you have a sensitive gag reflex, forcing yourself deeper can create another problem:
Your whole body may start tensing. You might tense your jaw. Then your throat. Then your neck. Then your shoulders.
Suddenly something that was supposed to be pleasurable has turned into a full-body endurance event. Stay at a depth that's comfortable and let your hand stimulate the rest. Your gag reflex is a protective reflex, not a personal failure you need to conquer.
6. Find a Position That Doesn't Wreck Your Neck
Sometimes the jaw isn't the only problem. Your positioning matters. TMD symptoms can include pain extending into the neck, and posture can contribute to musculoskeletal strain. Pay attention to what the rest of your body is doing.
Are you:
- Hunching your shoulders?
- Cranining your neck forward?
- Holding your head at an awkward angle?
- Propping yourself up with your arms?
- Clenching your jaw because you're uncomfortable?
- Trying to reach your partner instead of having them move closer to you?
Here are a few positions that can make oral sex more comfortable.
Partner Sitting on the Edge of the Bed, Couch, or Chair
Have your partner sit on the edge of a bed, couch, or sturdy chair while you kneel or sit comfortably in front of them. This can help because you're not hovering over their body or holding yourself up with your arms.
You can adjust how close you are so that your neck stays in a relatively comfortable position instead of constantly reaching forward. Add a pillow under your knees if you're kneeling, or sit on a cushion or low stool if that's more comfortable.
This position also makes it really easy to alternate between:

You can use a pillow under your head and adjust your partner's position until you can comfortably reach them without bending or twisting your neck excessively.
It might not work for every body or every bed setup, but when it does, it can be surprisingly comfortable.
The Best Position Is the One You Don't Have to Fight
There isn't one universally "correct" oral sex position.
If you notice yourself:
- Shrugging your shoulders toward your ears
- Hunching forward
- Holding yourself up with shaking arms
- Cranining your neck
- Clenching your jaw
- Constantly repositioning because you can't comfortably reach your partner
Change something. Add pillows. Move your partner closer. Have them sit up. Lie down. Switch sides.
The goal is to make your environment adapt to your body and not force your body to tolerate an uncomfortable position.

7. Ask for a Massage Beforehand
I'm officially making this part of the blowjob prescription.
If you're someone who holds tension in your neck, shoulders, or jaw, turn relaxation into foreplay. Ask your partner to give you a neck and shoulder massage first. You can also gently massage around your jaw if that normally feels good for you.
Gentle massage and stretching can be part of conservative TMD management, although people with diagnosed jaw problems should follow any specific exercises recommended by their healthcare provider rather than aggressively stretching the jaw on their own.
The goal before oral sex should not be:
"Let's see how wide I can stretch my mouth."
It's:
"Let's get my body relaxed before I ask these muscles to do some work."
And honestly? A slow massage that you both know is leading somewhere sounds like pretty fantastic foreplay anyway.
8. Check Whether You're Unconsciously Clenching
People with jaw tension don't only clench while they're sleeping. Sometimes we do it without even realizing it.
TMD self-care recommendations commonly include becoming aware of behaviors like jaw clenching and reducing them when possible.
During oral sex, occasionally check in with yourself.
- Are you biting down?
- Tensing your cheeks?
- Pulling your shoulders toward your ears?
- Holding your breath?
- Relax.
- Change positions.
- Slow down.
- Switch to your hands for a minute.
Giving oral sex doesn't have to involve maintaining maximum effort from beginning to end.
9. Use Lube So Your Hands Can Seamlessly Take Over
This is where lubricant becomes your best friend.

But persistent pain, locking, significantly limited movement, or difficulty opening or closing your mouth shouldn't be something you simply force yourself through. Those are recognized symptoms associated with TMD. And if oral sex consistently triggers significant jaw pain, that's worth mentioning to your dentist, doctor, or a physical therapist familiar with TMD.
Depending on what's causing your symptoms, conservative treatments may include things like physical therapy, manual therapy, exercises, massage, heat or cold, behavioral changes, and other individualized approaches.
Most importantly: Sex shouldn't require you to injure yourself for someone else's pleasure.
A loving, respectful partner will understand if you need to take a break, change things up, or stop altogether. Your comfort matters too.
The Best Blowjob Technique for TMJ? Make Your Jaw Do Less.
If you take one thing away from this article, make it this:
You don't need to figure out how to force your jaw to tolerate a 20-minute blowjob.
Figure out how to make your jaw responsible for less of the blowjob.
Use:
Your lips.
Your tongue.
Your hands.
Lubricant.
Sleeves.
Vibration.
Different positions.
Different sensations.
Breaks.
